New Student Athletes include Freshmen, transfers, & those participating for the first time on a Utica University athletic team. 
 
You will need to have a recent physical (Less than 6 months old) and physicians’ clearance for varsity sports participation. Review the checklist below to access forms and links.
 
NEW STUDENT REQUIREMENT CHECKLIST WHERE TO SUBMIT YOUR FORMS?
Required to attend classes.
  • Student Health Form
  • Immunization Record
  • Either waive or enroll in the University's student health insurance plans

*Access the portal by entering your Utica username and password then upload the requested health forms.

REQUIRED TO PARTICIPATE IN ATHLETICS WHERE TO COMPLETE?
  • Create Sportsware Profile.
    • Use Utica.edu email address.
  • Fill in all personal and insurance information on Sportsware profile.
  • Upload Primary health insurance card (front & back) to “Insurance” section.
  • Complete and sign 3 Forms in “Attachments section”
    • Release and Waiver
    • MHQ (Medical History Questionnaire)
    • Utica Concussion Education Plan
    • If you are under 18 you MUST print these forms and have a parent/guardian sign these along with your signature
  • Upload Sickle Cell Trait (SCT) Test Result *
  • Upload any corresponding medical records that you indicated in the history form **
Sportsware 
  • Complete baseline concussion test on campus
    • Information will come from your athletic trainer

Student Athletes taking medication for ADD/ADHD
o Download and have your treating physician fill out the attached form regarding the use of banned substances (i.e. Ritalin, Vyvanse, Adderall) in the treatment of ADD/ADHD

Incoming Student-Athletes: Use the new athlete letter below to answer any questions you might have. If questions persist reach out to your athletic trainer.

   * Sickle Cell Trait (SCT) Test Result
All new student-athletes (including transfers from non-NCAA schools, freshmen, & students who wish to try-out) MUST present proof of a Sickle Cell Solubility Test to participate on any of our intercollegiate athletic teams.
  1. Submit newborn Screening Test (SCT) Results:
  1. Contact your primary care provider (PCP) or hospital of birth/pediatrician's office and request a copy of your SCT status from birth. Most states required newborn SCT testing beginning in 1990.
OR
  1. Get tested
  1. You may use the NCAA sickle cell test site nearest your home to book a test through Quest Diagnostics (sicklecelltesting.pwnhealth.com).

** Medical Records
Our team physicians and the Athletic Training staff will need to review any medical records you indicated in the MHQ form. (e.g., physician's office notes, surgery notes, cardiology reports, clearance notes, etc.) Please contact your physician's office and obtain those records.
 
 
Returning Student-Athletes
 
RETURNING ATHLETE REQUIREMENTS WHERE TO SUBMIT YOUR FORMS?
Required to participate in athletics for 2024-2025 academic year:
  • Log into Sportsware Profile.
  • Fill out forms in “Attachments” section.
    • Annual Health Update
    • Release and Waiver
  • Physicals are not required for returning athletes.
